I would give zero stars if I could. My mother had been to 2 facilities one being Spring Meadows and it was a great place.. They took great care of her and then this place. It was a nightmare every day. The CNA’s I dealt with were so rude. I had eyes rolled at me because I was explaining how she liked to be positioned for the night.. then I had one tell me I needed to leave while they positioned my mom. I told them I would position her and she told me she could make me leave while they positioned her and I politely told her to leave my mother’s room. Every single time I went up there she was laying on her back where she had a stage 4 pressure wound. I provided a positioning wedge and a positioning pregnancy body pillow with a drawn diagram on it on how to position her so she wouldn’t be on her bottom. I asked them nicely several times to please use it and they refused to do it instead leaving her on her but with a stage 4 wound. Every time I went in I had to position her, wash her eyes because they were crusted and blood shot (I provided eye wipes for them but again they wouldn’t use them.. they were for demodex mites on her eyelashes.. she gets them because her stroke affected her eye blinking… her mouthcare was also never done and that’s extremely important because the stroke affected her swallowing.. one night I went up there and her oxygen machine wasn’t plugged in or hooked up to her trach machine (she later was hospitalized for low oxygen).. upon her return they put her in a filthy room that had pee and poop on the floor. I slipped on the pee.. they never once got her a call bell that she could tap if she needed anything and never put her call bell by her. And yes, I requested one.. but it was never done… one day she didn’t have water in her trach humidifier and she had an occlusion in her inner cannula.. and one day I went up there there was a pill on her floor by her bed that looked like her pain pill.. ( there shouldn’t be any pills on her floor because she gets her meds crushed in the hall and then mixed with water to put in her gtube. I finally pulled her out of there because she was literally rotting!!! Her wound was getting larger and she was being neglected. I reported all of this as it was occurring and nothing improved. The therapy team specifically the OT not the COTA she was amazing.. but the OT was extremely rude. I have a strong healthcare background and I would never treat a patient or a family member the was that some of that staff treated my mother and myself. It was a nightmare.

Terrible experience for my father at this facility. The first nurse on his shift never looked at his orders and came in as she stated she would after him being admitted. The second nurse didn’t come in until apx 5 hours after admittance and my prompting at the nurses desk. She explained to me twice she wasn’t even supposed to be working that night-someone had called out. Obvious staffing issues. His antibiotic medications were not administered in the evening due to pharmacy not delivering until 2-4am. Wound care not done until I complained. They would not get him a wheelchair-stated it had to be checked out in the am. Physical therapist had to assess him before they could help him out of bed to restroom. His wife and he checked out the next morning. It’s a shame because he really needed rehab services for the two weeks he was to stay. I would never recommend taking a loved one here!

Ive only been here a short time. I first got here on a Thurs night 8-9pm. A nightmare i didn’t expect. From hosp to here was getting dropped in the twi-lite zone. I’m part paralyzed, left on a bed with a call light. That has taken up to an 50 mins to answer. My wheelchair left at hosp, they’re all locked up here. Meds.a joke. I give them what i had, they give them back 1 shift takes and the other gives them back. Family had to bring more cuz the Dr who I saw 1st day hadn’t appr. them yet, It took me gonna call 911 for them to call Dr. My meds given back. They get mad on night shift cuz I won’t wet myself. The beds and bedpans have problems. So that’s caused them to roughly handle me. Floor gets slick in rooms from humidity, the motel heat/air can’t handle, and that’s filthy, walls are dirty, stuff flicked on, stuff run down them A true nightmare. New name previous AHC, same people. There are good employees just not enough. There are patients with out help.

Basically I got very good care here I just had a problem getting the right food. I spoke to Alicia and she spoke to her her staff and even put instructions on the paper that they are supposed to look at but they still didn’t do the right thing

I would recommend Park Meadows out of any of the other rehab facilities. The staff and the care Is wonderful!

The administrative team at Park Meadows Post Acute is truly impressive. They are organized, responsive, and handle everything with a high level of professionalism. It’s clear they care about creating a smooth and supportive experience for residents and their families.

My family had a great experience staying at this facility. The staff was kind and helpful and the facility was clean. Would recommend for future stay!

The staff is so good here! The CNAs are hardworking and compassionate, and the management keeps everything running smoothly. Plus they keep the place so clean! I felt very well taken care of the entire time and have nothing but good things to say!

I had a wonderful experience at this facility. Very professional, clean, and an incredible staff. Cannot recommend enough!

New company taking over was the best for the facility. We are hoping to see lots of great growth and changes.

Park Meadows has been wonderful! The staff provides excellent care and truly treats residents with kindness and respect. The Assistant Director of Nursing was especially helpful and attentive, making sure everything was handled professionally and with compassion. The private rooms are very nice and comfortable, which makes a big difference for residents and families. We are very grateful for the wonderful care provided here and would highly recommend this facility to anyone looking for a nursing home for their loved one.

Such a nice facility! The staff is friendly and helpful and you can tell they really care about their residents. 10/10 would recommend.
