At Park Meadows Post Acute, our goal is for every resident to feel safe and supported while living their healthiest life. Our team provides personalized care around the clock, and every room is private.
For many patients, we’re the step between the hospital and home after surgery or an illness. Others live with us long-term. Either way, our nurses, therapists, and dietitians review your progress every week, with the goal of helping you reach your highest level of independence.
Our Team
The care team at Park Meadows Post Acute is a group of skilled professionals who work together to help patients get back to their highest level of function. Each department below has a part in your care.
Leadership
Our licensed nursing home administrator runs the building. The leadership team also includes our medical director and each department head.
Therapy / Rehabilitation
Our rehabilitation director leads a team of licensed physical, occupational, and speech therapists. Each therapy plan is built around your needs.
Nursing / Clinical Staff
Our director of nursing leads our nurses and nursing assistants. A nurse practitioner also follows the vitals monitor readings from each room.
Social Services & Activities
Our social workers help you and your family plan for discharge. The activities team plans daily activities for a wide variety of interests.
Dietary Services
Our registered dietitian plans meals around each patient’s dietary and therapeutic needs and checks nutrition regularly.
Environmental Services
This team handles housekeeping, laundry, and building maintenance to health and safety standards.

PACS provides technology, systems, and business support to skilled nursing facilities like ours that elevate our local culture of care from a centrally-supported team of experts. As a PACS-supported facility, we utilize these resources to help improve our quality of care every day.
