Who We Are at Park Meadows Post Acute

At Park Meadows Post Acute, we provide person-centered care for residents and their families in a supportive setting between the hospital and home. We’re a 113-bed skilled nursing facility, and every room is private.

Some patients come to us while recovering after surgery or illness, while others live with us long-term. Each room also has a wireless vitals monitor followed by our nurse practitioner, giving patients an added layer of support during their stay.
